If you wish to cancel your contract, please submit your request by the end of the month in which you want to terminate the service. Additionally, if you wish to change options (apron or wet wipes), please submit your request by the end of the month prior to the desired effective month.

Parent Name Please enter the full name, such as "Taro Koto".

Child Name Enter the full name, such as "Hanako Koto". For multiple children, submit one application per child.

Phone Number Enter the phone number without hyphens. e.g., 0312345678

Email Address Please enter your email address.

Cancellation Reason Please select a reason for cancellation.

Other Reasons If you select "Other" as the cancellation reason, please provide specific details.

Final Month of Use Please select the final month of use.

Change Effective Month Please select the effective month for the change. Applications are accepted until the end of the month.
